WHAT HAPPENED
A fatality occurred during the unloading of fly ash at a customer site in mainland Europe.
While a bulk truck driver was unloading fly ash into the silo, the filter from the customer silo was projected into the air, flew over an adjacent silo, and fell down from a height of approximately 15 meters striking the contractor driver.

LEARNING POINTS / ACTIONS TAKEN
As the pressure in the silo increased while the fly ash was being unloaded, the silo filter separated from, the roof of the silo causing it to fall to the ground.
The silo was not equipped with a pressure relief valve (PRV) and the last delivery to this silo was approximately 6 months prior. There was no evidence of a preventative maintenance schedule to ensure the filter was cleaned and functioning properly.
