Incident alert

TRANSPORT - Fatal 6 - Debris stuck in a lorry's wheels was dislodged while driving on-highway and struck windscreen of another vehicle.

What Happened

 

A company received a complaint that a brick had fallen from one of its vehicles and damaged the windscreen of another truck when driving on the public highway. On investigation, using the 3rd party truck footage, it was evident that the brick came from under the company’s vehicle and not from the load area.

Further investigation revealed that the truck had run over some brick debris at the delivery point. A brick had lodged between the twin wheels on the drive axle of the vehicle, this was dislodged when the vehicle was traveling at speed on the motorway. This incident could have resulted in a fatality.

By working together, we can help to avoid this in future by asking for everybody’s help to keep our stockyards and traffic routes free of debris.





Learning and discussion points

 

The company issued the following:

  • Good basic housekeeping is a must in all our yards. We all look out for slip trip risks on a walkway, but now we are asking for your help to look out for the route the vehicles take to help keep these areas clear of debris.
  • Drivers, please take care when maneuvering on site to not run over discarded bricks.
  • If the above cannot be avoided and you are unable to move them, alert the yard manager or the nearest operative if you can.
  • If you are left with no choice other than to drive over debris, check the space between the twin wheels before leaving site and only if safe, remove what you can or ask for advice / assistance from your ATM.
