TITLE
FATAL 5 - Operative sustains impact to chest when struck by a manifold during a crane lift
COUNTRY OF ORIGIN

WHAT HAPPENED

A maintenance operative was struck by a manifold whilst it was being lifted by a crane to allow access to the screen deck. 

A permit and lift plan had been written.

During the lift the manifold became caught on a protruding bolt, the maintenance operative approached and levered the manifold off whilst the load was still in tensions in ‘lift’. When the load was freed, it swung back and struck the IP who suffered bruising to their ribs.   1. Clear assessment of the hazards must be completed prior to any job, all parties involved must discuss and agree, then sign the risk assessment and method statement. 
  2. Exclusion zones to be maintained to only allow designated crane team into the lifting area.
