WHAT HAPPENED
Safety Alert received from Build Safe United Arab Emirates:
A dump truck operater was tasked to unload sand at site. He started by activating the hydraulics to raise the tipper and hurriedly climbed out from his cab to release the latches at the back of the truck.
He was unable to release both latches in time.
As the load shifted to the rear of the skip and the centre of gravity shifted the front wheels of the tipper came off the ground. This could have caused the tipper to over turn.

Site vehicle risk assessments should be reviewed in light of the incident.
Review (and understand) truck operators manual.
Toolbox talks given to all drivers on safe tipping procedures.
Operators to ensure tailgate is unlocked before dumping and that vehicle is on reasonably level surface.
Banksman/spotter to be in attendance when any unloading is taking place.
Keep pedestrians and site vehicles apart.
