WHAT HAPPENED
A contractor was killed when a 4x4 road vehicle overturned on a quarry haulroad.
Although the operator was thrown clear during the incident, the roof of the vehicle collapsed.

Review arrangements to ensure that:
The risk of vehicle rollover has been adequately assessed.
Where a risk exists, any vehicle exposed to that risk has adequate protection for the operator in the event of an overturn.
If necessary, vehicles should be fitted with additional rollover protection such as rollover cages.
Seat belts should be fitted and worn.
