A New Holland tractor was travelling up the incline on a quarry haul road when the wheel hub failed causing the wheel to detach from the tractor.

An investigation identified the following root causes

• Improper extension of service life of equipment
• The hub assembly had operated for many years without inspection
• No formal checks in line with original equipment manufacturer (OEM) recommended service intervals
• Servicing had been moved in house with improper management of change.

• Service interval on Checkproof for tractor to include OEM specific checks.
• Identify all assets over 15 years old for immediate life extension inspections.
• Develop component life cycle matrix auxiliary equipment and implement annual health check on Checkproof.
• Service on Checkproof to include full-service document and annual health check document, with person responsible to receive alert.
