TITLE
Fatal 5 - A hammer falling 10 metres narrowly misses a team of workers.
COUNTRY OF ORIGIN

WHAT HAPPENED

A report was made that a hammer had fallen approximately 10 meters onto the electrostatic precipitator gantry. A scaffolding contractor stated the hammer landed approximately 3 meters from where the team were standing. They were unsure of where exactly the hammer had fallen, however following further investigation it was it was established that the hammer had fallen from the area highlighted in the image below.

Key Factors:

  • No injury was sustained from the falling object.
  • People had been working in the area from where the hammer is believed to have fallen from.
  • An employee who was carrying out housekeeping duties reported their hammer missing around the same time as this incident was reported.
  • An unsecured or poorly stored tool “hammer” has come off the edge of a platform and fallen during the housekeeping activity

  • Stand sites down and issue a TBT/briefing highlighting the requirement to use good methods of equipment storage, transportation, and tethering.

  • Consideration to be given to include tool storage in future inductions.
