Fatal 3 - TRANSPORT - Contractor falls from ADT steps and fractures his wrist

WHAT HAPPENED

 

Detailed below are the results of an initial investigation into an incident involving a fall from an articulated dump truck (ADT).

The IP was employed as a contracted mobile plant operator driving a Volvo A45 articulated dump truck at a quarry as part of a contracted crushing operation.

The IP’s primary role was operating an ADT, moving material from the contract crushing operation to stockpiles. The incident occurred in the morning when the IP moved the Volvo A45 ADT to the site fuel tank in order to refuel. After refuelling, the IP ascended the steps to the machine cab. At an approximate height of 1450mm the IP slipped, lost his handhold, and fell. The IP then got back into the ADT, drove it to the parking area, and then reported the incident to the site supervisors who arranged for the first aider to assess him. The IP was driven to A&E, where he was treated for a fractured right wrist and swelling to a knee .

KEY FINDINGS

  • During the investigation it was found that there was no risk assessment in place for re-fuelling the ADT
  • The IP was carrying gloves while descending the vehicle steps
  • The IP used unsuitable gloves for re-fuelling.

  • Consider if the following are in place for your sites:
    • Risk assessments for all tasks including suitable PPE
    • Suitable gloves at re-fuelling points
    • Roll out of slips, trips and falls TBT to employees and contractors
