TITLE
TRANSPORT - Tipper rollover at asphalt plant during delivery
COUNTRY OF ORIGIN

WHAT HAPPENED

A third party articulated wagon delivering dust to an asphalt plant overturned whilst tipping.

The driver was wearing their seatbelt at the time of the incident, so only sustained superficial grazes requiring on site first aid. It is suspected that poor load distribution may have contributed to this rollover incident.

The vehicle was safely recovered and returned to the haulier.

The company has set up an investigation team to understand root causes, recommend corrective actions and learnings to prevent any future incidents of this kind.

LEARNING POINTS / ACTIONS TAKEN

  • Review tipping operations on site, in particular for articulated wagons, including ground conditions, supervision, lighting and driver induction content.
  • Ensure all drivers are reminded of the importance of wearing seat belts at all times, including while tipping.
  • Discuss this recent incident with drivers as it demonstrates the safety critical protection seat belts provide while tipping.
  • Ultimately, drivers are responsible for their loads, but if they feel it's not safe or that something is wrong they should stop the activity and immediately seek site guidance and await further support and instructions.
  • Remind drivers of your company’s tipping rules or refer to rules included in the MPA  Driver’s Handbook – see image below from page 110.
