TITLE
Fatal 5 - Two large metal wear plates fell from a mixer via the discharge cone onto the loading bay floor
COUNTRY OF ORIGIN

WHAT HAPPENED

After a truck mixer pulled forward out from the loading bay after being stoned out, two large sections of steel lining fell from the discharge cone and onto the ground.

Had anyone been underneath the loading bay this had the potential to cause serious harm.

Investigation identified the steel wear plates were historic items, and had been tack welded rather than the use of a more permanent fixing.

The images below show the loading bay with the orange coloured cone, and the two steel lining plates that fell.

  • Ensure pedestrian access to loading area is restricted.
  • Review controls on site to ensure pedestrians in particular mixer drivers are not able to inadvertently enter the loading area.
  • Review inspection regimes to ensure the suitability of fixings is captured.
