TITLE
Fatal 1 – Isolation failure - Operative cleaning in banding zone was able to enter the adjacent bagging zone.

WHAT HAPPENED

 

An operative was cleaning in the block plant packaging line using a vacuum cleaner. The operative had followed LOTOTO and was cleaning the bander area. The operative was then able to take the tooling and himself from one zonal area to another as the tunnel through the enclosure guarding was not obstructed by a pack of blocks as it would usually be. The operative activated a sensor in the bagging zone which allowed the bagging machine to activate and the bagger frame to drive forward over the conveyor. The bagger then stayed in position over the conveyor awaiting the next sequential signals to lower. The frame did not lower, and no injuries occurred.

  • Ensure guarding is effective at all times and can stop persons entering into different zonal areas.

  • Product is not to be relied on as guarding between zones.

  • Ensure all procedures for isolation and work instructions are effective and all employees have been appropriately.
