Weigh Hopper Collapse Incident

WHAT HAPPENED
While batching 2m3 into a weigh hopper at Washington plant the bracket that the load cell is connected to gave way shearing the bolt causing the weigh hopper to drop and twist.
The area was Cordoned off and the plant isolated and shut down. Fitters were called who advised that we should fit safety chains to all our weigh hoppers to prevent this from happening again and to protect anyone in the working area.
Had someone been working in the area of the hopper, they could have been seriously injured or worse.

It is recommended that safety chains, or securing devices are fitted to the Premix weigh hoppers.
