A franchise haulier’s driver was seen underneath a raised tipper body with no body props, engine running, and was trying to clean out plainings from the hydraulic tank and vehicle gearbox while having the tipper body raised and unpropped.
A review of the root causes that led to this incident identified the following
Primary – The plainings/asphalt crew had left breakout in the road which caused the truck to get stuck momentarily causing the plaining machine to fire plainings over the front of the body and in between the body and cab.
