A franchise haulier’s driver was seen underneath a raised tipper body with no body props, engine running, and was trying to clean out plainings from the hydraulic tank and vehicle gearbox while having the tipper body raised and unpropped.

A review of the root causes that led to this incident identified the following  

Primary – The plainings/asphalt crew had left breakout in the road which caused the truck to get stuck momentarily causing the plaining machine to fire plainings over the front of the body and in between the body and cab.

• Drivers’ handbook to be altered to reflect the importance of NOT getting underneath an un-propped tipper body.
• Safety alert circulated to all haulers and drivers via email and MP Connect App.
• Night Distribution Resource Managers to pick up with all plainings crew and asphalt gangs the importance of not leaving breakout and sweepings in front of the trucks.
• Body props are only to be used by trained/competent LGV fitters with RAMS/ Management approval in place.
• Consideration MUST aways be taken to identify and control all forms of stored energy in all tasks to be completed (see list of types of stored energy below)
