LEARNING POINTS / ACTIONS TAKEN
This accident highlights the following issues:

• The cladding was only pop-riveted onto the securing brackets in 4 places at opposite ends of the roller door

• There was no centre securing bracket included in the design. The 2 pieces were simply riveted together for strength

• When the repair was carried out in 2004, a stronger method involving bolts should have been employed.

• The design of the cladding, with its flat top edge, allows spillage to settle and increase the load bearing weight to the securing rivets and brackets.
