TITLE
Serious Drill Rig Incident
COUNTRY OF ORIGIN

WHAT HAPPENED

A Drill Rig operative sustained a broken lower right arm as a result of entrapment on a drill rod carousel while changing a sensor.

The operative was taken to hospital where he underwent a 3 hour operation to repair the fracture.

The initial investigation has identified that the safety sensor (pad) in the seat of the drill rig had been disconnected allowing the operator to leave his seat in the cab and work on the drill carousel without appropriate isolation.

LEARNING POINTS / ACTIONS TAKEN

The following immediate actions were taken;

All site drilling activities were suspended.

Remove the drill rig from all operational activities.

Inspect all other drill rigs to confirm seat safety sensor (pad) is in place and functional.

Review and update Drill Rig pre-start checks to include the seat safety sensor (pad) is in place and functional.

Review all drilling operational procedures and re brief all Drill Rig operatives to confirm their understanding.

Issue an instruction to confirm: NO maintenance is to be carried out on drill rigs by Drill Rig operatives. Note: All maintenance activities MUST only be carried out by trained authorized maintenance teams.

Review Drill Rig operative lone worker procedures.
