Fatal 6 - Faulty isolators - Craig and Derricott

TITLE
Fatal 6 - Faulty isolators - Craig and Derricott
COUNTRY OF ORIGIN

WHAT HAPPENED

During a recent maintenance activity, it was noticed that the isolators (see images) could be locked in the ON position and also can be switched from the OFF to the ON position with a hasp and padlock applied.

The isolators were installed in around 2009 on a Benninghoven Asphalt Plant, they are manufactured by Craig and Derricott models DR063 3EB or 6EB in either 63Amp or 125Amp in 3 pole or 6 pole configurations. (please note the later model part number DR1256EB/RS is a revised design).

LEARNING POINTS / ACTIONS TAKEN


  • Please ensure that if you have this model they are replaced at the earliest opportunity and isolation procedures are modified for the plant to be isolated at an alternative source, until they have been replaced.

  • As part of your PUWER Assessment Please check all other isolators on the plant to ensure they cannot be locked in the on position.

  • Remind all operatives for the need to follow LOTOTO when carrying out Isolations on plant and equipment.
