TITLE
Conveyor Fire
COUNTRY OF ORIGIN

WHAT HAPPENED

During repair work by contractors at Central Regions Trentham Quarry in April, hot metal came into contact with flammable material on a screen under-pan igniting the field conveyor. The picture below shows the extent of damage caused. A hot work permit was in place, a hot work checklist had been completed, risk assessments and safe working practices had been prepared. However, the contractor altered their method of work without updating their risk assessment or informing site management. Quick action by site employees and the contractors to isolate electrical and gas cutting equipment prevented a more severe incident.

LEARNING POINTS / ACTIONS TAKEN

Remember:
- All Rubber products burn vigorously when ignited.
- All sources of ignition must be controlled when working on or near rubber and rubber lined products, these include:
•Naked flames
•Cigarettes and matches
•Welding operations
•Process heat and flames
•Space heaters

Precautions for Hot Work near Rubber products/linings:
•If possible remove the rubber product or lining.
•Ensure sparks or hot debris is contained, even after the work is finished. Wet the work area before and after hot work.
•Water fire extinguishers must be available.
•Post a competent “Fire Watch”
•Prepare an Emergency Plan
•Take care if using solvents or adhesives.

Ensure all Rubber lined equipment is marked with the signs below.
