Always ensure your pre-start safety checks are completed thoroughly, pay close attention to load and stress points. Always ensure your mobile plant is inspected in accordance with the manufacturers recommendations.
The HM400 (above) was removed from service until the tail board could be removed safely.
Komatsu have been notified that a retro fit part fitted as a result of a previous alert has also failed. Komatsu are to investigate.
