Cement Truck Loses Tyre
Safety Alert received from Build Safe United Arab Emirates:
A rear nearside wheel broke free from a cement truck.
On investigation it was established that some of the retaining nuts were loose and previously nuts had been tightened directly to the wheel boss. Chamfering wear at the stud orifice increased over time until the studs failed.
There was also a mix between star plates and washers indicating an inconsistent approach to maintenance.
