TITLE
Operator trips on lifting hook on hatch - Slips, trips and falls

WHAT HAPPENED

Whilst rigging wash-down hoses the IP tripped on a lifting eye on the forward main deck pump room hatch, spraining the right ankle.

The IP was rigging wash-down hoses following paint preparation works on the forward main deck. The hoses were rigged and the hydrants opened. The IP noticed a kinked hose so he made his way over to de-kink it. Concurrently, the 1st Engineer was preparing a routine pressure test on the emergency fire pump and requested that the pump be started. The fire pump was subsequently started from the bridge. As the hydrants were now open and the hoses unmanned, the hoses started to flail. The IP moved towards the hose in an attempt to gain control. In doing so, he tripped on an un-marked lifting eye on the pump room hatch, causing him to fall to the deck, spraining his right ankle.

  • Prior to conducting any task, ensure that the risk assessment is suitable and sufficient and specific to all stages of the task

  • Review trip hazards across the vessel and ensure they are painted yellow

  • Review risk assessments which include the use of water on deck.
