TITLE
TRANSPORT - Fatal 2 - Distractions and pressure of work - readymix driver forgets to detach water hose from rear of his vehicle.

WHAT HAPPENED

CEMEX has shared this incident that illustrates what can happen a when a driver is distracted or feels under pressure. The PowerPoint / video summaries potential causes of distractions or pressure at work.

It then explains what happened when driver of a readymix truck drove off with his water hose still attached. Fortunately nobody was injured in this incident.

Key learning points

  • A pedestrian in the area could have sustained a significant injury. - Where possible remain in your vehicle or stick to designated walkways
  • Following the incident the driver admitted he was distracted as he was waiting results from a test related to a health issue
  • It is essential that we are alert to hazards and risks associated with our work at all times. Please speak to your manager if there are any issues, distractions, illnesses or medication that could impact your safety, so that appropriate support can be provided.
  • If you notice a colleague is not their usual self, either distracted or not their usual self, and are concerned for their safety, encourage them to speak to their manager
  • Where necessary, in the event of a significant safety concern, STEP IN and raise the matter confidentially your manager. It is important that we all look after ourselves and our colleagues.
  • By sharing this HiPO, the local operation, helped prevent similar incidents occurring elsewhere. Remember we are all safer when we share incidents and learn from others
