TITLE
TRANSPORT - Reversing vehicles and pedestrians - A reversing vehicle nearly collided with a driver
COUNTRY OF ORIGIN

WHAT HAPPENED

A mixer truck was reversing to park up. The space chosen to park, was between a bulk cement tanker undertaking a delivery and a mixer truck that was being loaded.

As the mixer truck reversed, the driver of the truck that was being loaded walked from the rear of his truck up to his cab, which placed him at risk of being struck by the reversing vehicle.

On realising the reversing vehicle, the driver quickly retreated to the rear of his vehicle. The reversing vehicle stopped, before a collision occurred.

Image 1 - Driver of mixer truck being loaded approaches his cab as the other truck mixer reverses into parking position.

Image 2 - Driver begins to access his cab and then realises how close the reversing mixer truck is coming.

Image 3 - Driver dismounts and begins to move away as mixer truck comes very close to him.

LEARNING POINTS / ACTIONS TAKEN

  • Review site traffic management; are parking areas away from operational activities.
  • Consider maximum numbers of vehicles allowed on site. If busy where can vehicles park up awaiting to be called forward to site.
  • Stop work/production where traffic congestion is increasing risk.
