An articulated dump truck (ADT) operator was tipping wet material at a designated tip area, the tip had become full and this resulted in them reversing down and attempting to tip on a steep access ramp. The ADT’s inclinometer flagged the position as unstable. The operator moved forward to reposition, which caused the wet material to shift in the skip. After applying the tipping brake and selecting neutral, the operator was preparing to tip when the cab unexpectedly lifted and rolled over.

Violation of given instructions - Driver of ADT did not follow site tipping RAMS, task RAMS stated tipping on level ground.
