TITLE
Articulated dump truck operator puts himself at risk of serious injury

WHAT HAPPENED

 

An operator exited an articulated dump truck in an unsafe area with no hard hat, gloves, or safety glasses. He stood between the cab and body of the vehicle below the skip to urinate whilst it was still being loaded by an excavator. This was reported to site management by a member of staff using the Near Hit QR code, it was not picked up until much later in the day.

Once the incident had been picked up by management, both machines were stopped and the operators brought down to the offices for an investigation. The articulated dump truck operator then received a positive drugs and alcohol test and left the site. The excavator operator tested negative and had been unaware of the incident occurring and would not have been able to see the other operator due to the position of the mobile plant.

KEY FINDINGS

  • The operator was operating while under the influence of drugs which may have altered his perception of risk.

  • Ensure the use of the QR Near Hit reporting tool is continued to be used for near hits.
  • Ensure that everyone understands the importance of reporting severe near hits to a manager or supervisor immediately—it's a vital step in protecting people on site.
