While filling the mobile fuel bowser with diesel from the main site tank, the fuel delivery hose began to leak. This resulted in diesel spraying from the hose and coming into contact with the hot exhaust of the fuel pump engine, which ignited the fuel and started a fire. While trying to extinguish the fire the IP suffered a small burn to the back right hand.
The outcome of this incident could have been far more serious.
ROOT CAUSE
The diesel fuel delivery hose from the static tank that delivers fuel to the bowser was damaged in some way and caused a leak. This was not spotted before fuel delivery commenced.
