An articulated dump truck (ADT) was hauling material from a train to a stockpile in the depot when the wheel nuts on the central rear nearside wheel failed, causing it to fall off.
An investigation revealed that the wheel hub had been replaced the previous month but it had not been retorqued following the return to use after the repair. The service sheet stipulates that it is the responsibility of the user to ensure that the assembly is checked and retorqued if necessary.
